My nose is almost constantly running to a point where I have to do that sniffing thing to pull everything back up. Even when I feel like it’s all clear and can feel the air running through, it returns in a few minutes.
If I don’t do anything it’ll keep dripping down to the top of my lip. It’s always clear and a little watery-like, never a yellow sludge, and there’s never any boogers.
Recently I’ve tried sniffing these which temporarily solves the problem, but it still returns later.
Is this just a normal thing nobody talks about? It’s very annoying

So the situation has (surprisingly) gotten a bit worse now.
I'll continue the points from where I left off:

  1. Since March this year, I've been sending in at least 5 applications per day for trainee, junior and experience requiring positions. They've all involved tailored CVs and personalized cover letters after taking the time to research the companies. The vast majority of them get replies like "We chose candidates whose profiles were a more perfect match for the position" without any explanation or feedback. I wouldn't be surprised if they're making "culture fit" assumptions at this point. The 1% of them I do get interviews for end up ghosting me after any mention of having to deal with the Ausländerbehörde for getting permission to work. There was this one time I made it to a 2nd round interview, but it didn't work out because they wanted someone with more "cold calling experience".
  2. I got my Fiktionsbescheinigung extended till late June, although it still says "Erwerbstätigkeit nicht erlaubt" - something that seems to cause employers to ghost me as soon as they see it. Even the Arbeitsagentur said they can't help me unless my situation with the Ausländerbehörde is solved.
  3. I met a couple of lawyers for Arbeitsrecht and Sozialrecht, who basically didn't get to the point where they look at proofs, and told me it's too late to get any compensation regarding my health condition. I brought about a dozen pages of info to show them, and they barely even discussed two.
  4. On my lawyer's recommendation, I got a new doctor's certificate stating the maximum possible recovery period (until late July). I still have a pending checkup with the hospital's neurologist later this month to see if the tumor has a chance of coming back, and appointments with the ENT to discuss my hearing loss are available directly in September. External ENTs say only the hospital has knowledge about the nature of my operation, and that only their specific ENT can take care of it.
  5. A migration counselling center discussed my case with their own lawyer, where the conclusion was that the Ausländerbehörde had wrongfully stated "Erwerbstätigkeit nicht erlaubt" on the Fiktionsbescheinigung. I discussed this with my lawyer and asked them to talk to the Ausländerbehörde about it.
  6. Now I get a letter from my lawyer that the Ausländerbehörde can't extend my stay further, their argument being that the operation was "after my work search permit expired". They're completely ignoring the doctor's certificate which clearly says my symptoms were making it impossible to look for a job during this period, and their argument for not changing the words on the Fiktionsbescheinigung is something along the lines of "I'm unable to work till late July anyway, so there's no point". They've asked my lawyer to decide by June 15th whether I want to withdraw my residence permit application in exchange for a border crossing certificate lasting until July end (with the possibility of them examining a work contract in case I manage to get one by then), or whether they can go ahead with the rejection. Here's what they wrote in their email to my lawyer:

Wir können hier gegen Antragsrücknahme nur insoweit entgegenkommen, dass wir bis Ende Juli 2023 eine Grenzübertrittsbescheinigung ausstellen.
Sollte innerhalb dieses Zeitraums ein Arbeitsvertrag vorgelegt werden können, werden wir diesen Antrag prüfen.
Andernfalls können wir lediglich das Ablehnungsverfahren vorantreiben.

I'm looking for opinions on why they're being so aggressive and unhelpful here.
Is it because they know they screwed up and want me to drop the case myself so they don't get found out eventually? Are they hoping that I'll crumble under time pressure and won't get to prove anything if further treatment is required?
Is it because they actually believe I'm lazy and not even trying, since they lack the perspective of a non-EU person (from somewhere that isn't a first world country)?
Is it just a result of the case handler's personal need to power trip by making the "Ausländer" feel small and powerless?
Or is the Ausländerbehörde actually bound by "rules" they supposedly can't make exceptions for in such cases? This being despite the fact that I'm not taking unemployment benefits at all and the only problem they would have with me is the fact that I'm breathing their air :/

Honestly this is all just way too stressful, and it certainly doesn't help with the recovery.
Any thoughts on how to proceed with this?
And would a "rejection" be a major hurdle in the future in terms of immigration?

We surveyed users back in late March about their thoughts on the Subreddit. This is part three of that survey. Anyways, let's dig in! Here are the “other thoughts” you guys wanted to share. Well, some of them at least..... (I'm not posting the response that was just "poi")
  1. “I hate subs”. Cool story, bro. I think they’re okay for a lunch, but I’m more partial to a hamburger myself.
  2. “The CV/Subs are bad posts should be considered as low effort posts. They're mostly just for karma farming” . Personally I agree. I strike down a LOT of these posts for the low effort rule, but I’ll try to be around more to catch as many as possible. There were at least 20 comments that had a similar sentiment.
  3. We had a lot of people who suggested pinned sticky threads for all kinds of things. I highlighted over that in the last mod post about the survey, but here’s the TL;DR.... It can’t be done with Reddit’s functionality. It’s rare that we have a free sticky spot to give to something. Reddit only gives Subreddits two, because they’re too busy trying to kill off third party apps instead of QoL improvements.
  4. "Wasn't too pleased that a thread about the Haida auction was removed for the lamest of reasons. Smacked of WG shilling - the discussion was on point about the cost. Mod that was responsible for that should be removed.” You were addressed in modmail, but I did some digging. This was an incident that happened a few months ago (You can go through patchnotes if you really care, I guess). Anyways....your post was removed for being a duplicate. There was indeed another post about the auction... which was removed by a different moderator at roughly the same time. It’s called human error, and it happens sometimes. We’re not machines, we’re just humans. I know that amongst the mod team, we have two EMTs, a farm equipment dealer employee, an IT guy, a volunteer aboard a real life warship museum, and a PR guy for a large company. Those are the ones that I know- I’m sure there are TONS of other cool mod jobs that I don’t. The point is that we’re humans. We all have jobs to put food on our tables. We do this, because we love the community and the game. We’re not going to be as perfect as a salaried person would be, but that’s also why this place is as great as it is. Please be patient with us, and don’t fill our modmail with shouting when stuff like this happens. No moderator will be removed over this issue.
  5. We did have one user who accused us of being paid shills for Wargaming, because there’s always that one person. I just want to know where the heck my checks are then. Come on Wargaming, I’ve been here for like 5 years. How long do you need to mail a darned check? I could use a new set of tires for my Saab, so hurry it up, will ya? :P

I don’t have a lot of time to play the game anymore- I’m basically a full time workaholic. But, it’s cool to watch someone nail a double strike, or see what changes are coming to the game. It’s a way to be involved and enjoy the game, without having to click a battle button.
  9. "Have you a moment of time to talk about our lord and saviour tiger 59?” Nope, I do not. Thanks for asking, though.
  10. "Tsukiumi-Chan deserves a Fujin review, for the memes.”. Thank you, and I agree. After 3,500 battles in it, a review would be nice. But, maybe the friends we made and the torpedoes we launched were the meme all along. Or something like that. I don’t do catchy sayings very well. I just torp things.
  11. "Thanks for the effort in keeping this community alive. The game may have gone down the shitter since release, but the subreddit has always been a great place for info, guidance, and inspiration to keep playing the game.” We’re glad to be here for you all! There are times that the game and the Subreddit can be trying to our patience, but we’re glad to be here. I remember when I first downloaded WoWS, back after it just left Beta. It’s been a lot of fun along the way, and I hope it will continue to be, for many years to come!
  12. "The constant, CVs suck, subs ruin the game posts that show up 5 times a week, or more, are getting really, really tiresome. Make a day where everyone can focus on posting why they hate class/ship X,Y,Z and ban such posts for the rest of the week. Call it "moaning Monday, "whining Wednesday" or "frustrated Friday" or something like this." . A weekly thread to complain may not be a bad idea, actually. I’d be interested in seeing what you guys think about the idea. We may have to try that out. You see, stuff like this is why we do these surveys. Oftentimes, you guys come up with ideas that we just wouldn’t even think of

  14. "NERF SUPERSHIPS, MATCHMAKING +1, -1. DO SOMETHING FOR TIER 1-4, ALLOW THEM TO COMPLETE THE DAILY MISSIONS(BATTLEPASS ETC.)". Like half the responses were stuff like this. I just want to remind everyone again that this is NOT a survey for Wargaming. There’s nothing we can do about in-game stuff. I can nerf superships by playing my Satsuma really badly if you’d like, I guess.
  15. "What happened to the original sub description? Go back to the one about never having submarines.”. We talked about this for awhile. We felt that it breeds toxicity. Imagine if you’re a casual player who likes playing submarines. Would you want to interact with a place that said that in the description? You don't have to like subs, or carriers, or the color blue as a ship camouflage. But, you need to realize that sub and CV players are people too. They deserve the same respect that anyone else does.
  16. "Need to have a weekly sticky for advice/general chat. Possibly rotating daily themed stickies like a bragging one to talk about your awesome achievements over the week, or an Ops thread.".Ignoring what I said about stickies earlier, I would like to see more themed threads, personally. I haven’t asked the other moderators about it. The problem is that they take time to make, and we just frankly don’t have the manpower to do it reliably. I can program AutoModerator to do them, but that feels a bit too cold and impersonal to me. Your thoughts? We have tried all kinds of things over the years. Basically: If there’s work involved, it never gets done. I used to help write “Ship of the week reviews”. That worked great until I ran out of ships I knew enough about to spotlight. We tried a video replay of the week feature.... which led to people whining about the ship choice, or the YouTuber who uploaded it, or other things like that. We just couldn’t make everyone happy. We’ve tried working with content creators directly, and many of them would stop submitting after awhile. Basically, if there’s work involved, we will need help. Especially going into fall. I run a custom baling business with my father, so all summer and fall, I pretty much live in the cab of my tractor. I can’t write Reddit posts reliably when I’m doing all that. I wish I could be here more.
  17. "Overall I think the sub is doing a lot better than for instance the /worldoftanks sub, so the moderation is IMO doing a good job. The overall mood is going to be negative at times because the game is heading in a direction a lot of people don't like, and I don't think that should be censored (and I don't think it is, I'm just saying we shouldn't start). It appears that this subreddit is one of the few places where WG employees can actually get some feedback occasionally and our voices should be heard." . There were about a dozen comments like this. I don’t really have a way to respond to this, but I felt it should be shared.
  18. We had one person who asked what happened to all the Depeche Mode references I put in the modposts. This is a bit of a silly question, but it’s a good one to end on. We haven’t done that for years. It’s a good reminder that while we may grow and expand, a lot of us have been here for the long haul. That shows how much we care about the game. The moments we’ve cried, laughed, and raged against HE slinging Kurfursts. This is a game after all. Have fun with it! As for your question, don’t worry. We’ll work in more DM references. As for when? It’s just a question of time. What? You can’t be too surprised that I worked one in there. There’s always room for one or two. You can’t overdo it though. You have to get the balance right.

2023.06.03 14:56 TiffanyBlue0ABAB5 Sometimes the right brush helps (journey from dry brittle hair to pretty darn decent hair)

Like many of you here on this sub, and other hair care subs, I had an issue with my hair and couldn't resolve it. I tried multiple solutions and finally found one that worked for me and I hope this can help you.
My hair: Type 1B or 1C when it's shorter and 2B when it's longer (past shoulder length). It is greying around the temples and underneath. I had some wonderful highlights done which were screwed up when my colorist called out sick and the salon felt comfortable allowing someone else to do my next session. My hair felt like doll's hair - super stiff and plasticky. They tried to fix it in house, but couldn't. All my money was refunded and then the salon asked me to not return after they screwed it up so bad. I had to cut off over 10 inches. Found out later they allowed the receptionist to apply and rinse out my toner and hair treatment while the colorist took a trip to Starbucks for a Frappuccino, I didn't know at the time, but found out after the damage was done. Right now it finally made it passed 'bob' length and almost to my shoulders.
Covering the grey: After cutting off all my hair after that disaster of a color mishaps, I decided to go back to my natural hair color while growing out, grey hair and all. As it grew, I would get trimmed off what remained of the hair dye/colored part and really tried to take care of it. When it finally got to bob length I decided to go back in to get the roots/grey covered up. After two different sessions of the colorist 'warming up' my color, which I never asked for, I decided to just do my grey areas at home on with a Demi permanent color about every 2 months on my roots and just the grey area.
Problem with the ends: I was still having a LOT of issues with the ends of my hair though - despite all the deep conditioning masks, using hair oils, etc, it just was SO brittle and dry on the ends only. I've always had a super oily scalp and dry ends. I made the mistake of listening to advice from a hairstyling YouTuber and spent a waste of money on Redken's Volume Injecting Shampoo (seriously, if you have medium or thick hair, avoid....this guy said to use it on medium hair). Same stylist said to use a leave-in condition, which actually did help, but not the brand he recommended. My ends were just SO tangled and dry.
How I tested and failed and solved this issue:
- I have oily roots. As much hate as there is for sulfates, my roots need sulfates. I always always shampoo twice. I started to use a sulfate shampoo first to really clean my scalp and shampoo with a non-sulfate shampoo second. If my hair is really greasy or oily, I'll shampoo twice with the sulfate shampoo.
- Condition in the shower but I also use a spray leave in conditioner after it's towel dried. I found less is more for my hair overall, BUT I do spray extra on the part of my ends that tangle. I found using a gentle vent brush to go through my hair afterwards helps distribute.
- Blow dry spray is key - again, I found out through trial and error that I don't need a lot, just a good light mist. I used to air dry my hair ALL the time, but it got thicker and more tangled when I did this and I could tell my hair shaft was swelling up and staying that way. I found it was more damaged - plus it takes over two hours to air dry and I cannot show up to work with sopping wet hair. If I shower at night, I find this is okay, but I try not to wash my hair at night. There are some of these combined with leave-in conditioners that work well for my hair too.
- THE BIGGEST CHANGE - using the right brush to blow dry. I used to just use a flat vented brush or a paddle brush while blowdrying my hair. It looked just fine, but it didn't have a lot of movement and still felt stiff regardless of what products I put in my hair. Shifting back to a rounded blow dry brush was the biggest game changer. I HATE using it and I'm going to state that. My arms get tired, I start sweating while using it because my hair is thick and it takes effort, I have to work in sections, but damn it makes my hair smoother and shinier and my hair moves! I can run my fingers through my hair without tangles and it feels good. I only blow dry and style on the day I shampoo and wash my hair. I don't use heat on the other days, I just adjust how I wear my hair that day.
Caveat - I don't like that massive one-step hair dryeround brush that is all the rage a few years ago. I found that particular tool did way too much damage to my hair because it seems to be hotter. Just a plain hand-held round brush in the largest barrel for your hair if I plan to wear it close to straight. There are a few smaller barrel ones that are meant for styling only in a lower heat setting, I use those at the end only, not for drying my hair.
- Hair oil - Using hair oil on my ends after blowdrying and using what little bit is left on my ends to go over my baby hairs up front has made a difference. I also put a bit of hair oil on my ends only about an hour before bed. I don't use a lot, just enough to prevent drying out.
- Hair oil before shampooing or overnight. I've read this on this sub and others and it actually works wonders. I use the OGX extra strength coconut miracle oil for this, but I'm sure there are others. I've used the Kerasate and even Argan oil. I prefer the coconut one for the price point. Either 2 hours before shampooing, best over the weekend, or the night before, put a lot of oil in your hair on the mids and ends. If doing the night before, either be prepared to change your pillowcase in the morning, or sleep with a sleep cap. When shampooing out, you do need a sulfate shampoo or clarifying shampoo to help wash off the excess oils for the first shampoo. Doing this once a week has really helped.
- Slumber cap. This has helped tremendously. There are loads on Etsy and amazon. I have a favorite I found off Etsy (as an example only) because I like it's adjustable. You can look and choose one for your head and hair length. I cannot speak enough how much a silk or satin bonnet for sleeping has helped with my hair health.
- Limit dry shampoo. Since I have oily roots, I can maybe go three days before I have to shampoo again, even using dry shampoo.

My favorite products that have worked for me during all this. They do range in price point and I use drugstore and higher end.

Paul Mitchell Shampoo Two
John Frieda Blue Crush for Brunettes <- only when my hair starts getting brassy
Kristen Ess Signature Shampoo
Trader Joes Tea Tree Tingle
Redken Hair Cleansing Cream <- omg, HG. I bought the mini size to try and a month later I still have product in it, you DON'T need a lot. Worth it.
L'Oreal Ever Pure Bond Strengthening Shampoo <- new find, very happy with this so far
Kerastase Bain Satin 2 <- I use this mostly in the winter

Conditioner:
Matching conditioners to John Frieda products
Kerastase Nutritive Masquintese irisome <- have purchased for over 6 years. You only need a half walnut size glop even with longer hair. Less is more.
Hask conditioners in those packets you get at CVS/Walgreen. I can usually get two uses out of each packet
Nexxus conditioners in those packets, I like those too. Basically any deep conditioning drug store packets, even the keratin ones I find are good for a once a week or once a month deep conditioner

Dry shampoo:
Eva NYC Freshen Up <- have repurchased for years. I have brown hair and it doesn't leave white marks
Not Your Mother's Clean Freak

Leave in conditioneblow dry spray:
It's a 10 Miracle Silk Leave in
L'Oreal Ever Pure 21-in-1 color caring spray <- new find, I love this one

Hair Oils:
Verb Ghost Oil
Olaplex No.7
OGX coconut miracle oil
Kerastase Elixir Ultime <- lasts forever HG
Paul Mitchell Smoothing super skinny serum
Hask Argan Oil

Hair Cream:
Olaplex No. 6 (I mix this with the Paul Mitchell skinny serum sometimes, it's fabulous)

Brushes:
Honestly, they are all Conair brand from Target. My oldest one is hitting 6 years.

Round brush with hot air tool (doesn't get super hot for styling only):
Conair Infiniti Pro

Covering up those grey hairs without resorting to box dye:
Moroconnoil Color Conditioner in Cocoa (there are other shades) - use a hair color brush to apply to grey. Leave on about 15 minutes then add more to the rest of the hair to even out the color. The grey turns more of a light blondish color, but blends well.
Christophe Robin Shade Mask in Ash Brown - this does ashy brown SO well, but damn it's expensive.
John Frida's Brilliant Brunette Collection. Great if you want to go a bit deeper. Again, apply to the roots first and wait, then work through the rest of the hair.
Color Wow Root Cover Up - okay, I made fun of this at first but it's become a favorite now. If I pull my hair back and got some grey poking through around where my hair meets the top of my ears or want to tidy up the part in my hair, this stuff works great. AND it sticks around if you don't shampoo the next day. Once you get the hang of how to apply it, you can do it fast and it honestly does look pretty natural.

Hope all this help you on your hair journey. My goal is to grow it back to bra strap length. Since we're going to be moving in a few months I'm putting off going to a salon for color touch up and wait until we get to our new location.

2023.06.01 20:46 Altruistic_Aerie_479 NEVER buy CVS Pregnancy Tests (False positive)

Last week I had the worst pregnancy scare of my life. I was 4 days late for my period (not out of the ordinary for me to be irregular a couple times a year) so I decided to take a CVS brand digital pregnancy test CVS Test since I already had it laying around (it was set to expire in 2025 so I was in the clear).
It comes back positive and I immediately drink some water and take two others which turn out negative, but I also figured I could have diluted the hormone by drinking.
I get a blood test the next day and it comes back negative but I am so so upset with the quality of these tests and I later find out that a friend of a friend is in an IVF facebook group and they talk about how bad CVS pregnancy tests are and how their doctors advise against them.
Will be using name brand pregnancy tests only from now on thank you very much!!
Feel like CVS should be held accountable for traumatizing us women with their inaccurate tests - even the reviews on their website speak of false positives.

What is the difference between a Cash account and a Margin account?
A cash account means you must pay for your securities 100%. On the sell side, you must maintain enough funds to cover your entire max loss.
Conversely, in a margin account, the broker will only require you to have a certain percentage of the cost in the account, and will loan you the remaining balance. On the sell side, they will require you maintain only a certain percentage of your potential max loss. The amount they require will increase if the position moves against you. If it moves against you to the point that your required margin amount exceeds the cash in your account, a brokerage will margin call, telling you to deposit more funds or they will close (buy back) some of your positions. Interactive Brokers says they do not make margin calls, but they do send you notifications.

What are the Margin requirements I see inside of Trader Workstation?
When you view the order preview pane, you will see the amount of initial and maintenance margin required to take out the position. It will show the change to your account. This is up-to-date but impractical in seeing the "big picture".
The IBKR help staff may also tell you to view the margin requirements on the description page of the stock. These can be WRONG and should not be relied upon as they are not kept up to date by Interactive Brokers.
The help staff may also refer you to the margin formula page. These may also be wrong as the formulas may not be up to date, and should not be relied upon for calculation.
There is no way to see accurate margin % inside of Trader Workstation (TWS). We can only see accurate dollar amount of a chosen position by selecting it and pulling up the order window pane. There is no way to compare margin inside of Trader Workstation unless you pull up each position one at a time and check the value in the order window pane.
To accurately see margin percentages and compare it from one position to another, we can achieve this by accessing their API and using ib.WhatIfOrder. I have done so and posted the results below.

Cash Requirements for Selling Naked Put Options:
The data here was pulled on June 1, 2023. This shows you how much of the potential max loss (strike price * 100) that you would have to contribute to sell a naked put option that is "At-The-Money". you'll notice these are significantly different than what is shown in the software for some of these tickers.
You can see on the chart below that any stock that was caught up in the "meme stock" volatility of the early 2020's has been cranked up to 100%, meaning you will have to deposit enough cash in your account to cover the full potential loss before they will let you sell it. Popular volatile tech stocks such as TSLA and NVDA are higher as well, around 50%. Generally, anything that has steered clear of the volatility is still around 30%, which is the default amount. The indexes are a bit below that, with SPY at 20% and SPX at 15%.

Also note that you get some assistance in that the premium of the option goes into your cash account immediately after the sale. IB does factor in the premium in determining whether you meet the margin % requirements, meaning the actual amount of cash you will have to deposit might be a little bit less. On the graph below, you see COIN's required margin % has been overridden to 100% for every option contract, but when factoring in the premium from the sale, the blue line shows that you'll only have to deposit 95% of the potential loss for an ATM put.

Call options work a bit differently in that there is no max potential loss. The loss could be infinite in theory. Therefore, the amount you have to deposit can exceed over 100% of the (strike price * 100)

We can take a look at COIN again and see the amount is much higher than 100%. It costs double the amount of cash to sell a call on COIN ATM than a put, which destroys the profitability of a call in this scenerio.

The week and weekend leading up to Memorial Day was not as quiet as expected. Our clinic buzzed with last-minute occupational testing, worker's comp cases, and unexpected injuries so severe that I thought we might transfer the patients to the ER. Luckily, only one or two fit that category.
Patients are still waiting too long to seek treatment. We saw a patient with a dislocated shoulder that suffered for a week before seeking treatment. More cellulitis patients arrived. I caught an article about it written by - of all people - Melissa Gilbert (Little House on the Prairie). Eye infections, styes and chalazions were up. No bilateral (both eyes) infections showed up.
Sinus pain and pressure were common complaints, with just one or two reporting yellow or green "alien" mucus.
On the Covid front, I heard they had three Covid cases on Monday, but we had just one or two this week and weekend. This may be due to the fact that most patients are now refusing COVID testing. The vast majority of them ask for Strep testing, and only a few resulted positive for Strep A. The rest were put down to "unknown virus", or "viral syndrome" (in the very young). To say that I am disappointed that so few chose to Covid test is an understatement.
Of course, one of those positive cases was given to me to discharge, and to my great relief she was at least directed to wear a mask as she sat at my desk. The problem? She leaned into the window so closely that I nearly felt the need to back myself away. I resisted it, knowing that the cloud of viral aerosolized particles was not directed toward me; but I hastened her paperwork and excused myself from the scene as quickly as possible. Sometimes it's better to just let the air clear, so to speak.
None of my coworkers are wearing masks anymore, not even my fellow front office workers. I overheard a conversation between a supervisor and some unidentified person (I don't eavesdrop, but when my name is mentioned I tend to pay attention). It seems they were talking about how well the dropping of mask-wearing was going. I was mentioned as being "proactive" versus everyone else, and that things seemed to be running smoothly...but I am not entirely of that opinion. In fact, I was somewhat angry that I had been singled out as a representative of a class of people; it had a ring of political side-taking even though the words were carefully chosen.
Several patients who were obviously ill with respiratory ailments refused to mask. I insisted, prompting scowls and grumbling, but my coworkers gave up, and said nothing. This in turn caused other patients to insist upon waiting in their cars. The divide between patients who just don't care about spreading disease and those who don't want to catch any seems to be developing quietly. While some people won't wear masks, they are still interested in separating themselves from obviously sick people. I do wish they would understand that Covid can be carried by even "healthy" looking people. Three years in, and they still haven't received the message.
When I look out over the crowd of unmasked patients, I see vulnerable seniors, babies, pregnant women... I feel that our signs should say, "Enter at your own risk."
I had a conversation with one of our doctors about Paxlovid when a middle aged couple came in asking for it due to symptoms (feeling "Very sick, nauseous, with a fever,") and a positive COVID test. I prefaced my question to the doctor with, "As I am sure you have heard that the FDA formally declared Paxlovid a safe and effective treatment for COVID," and then followed up with, "Are you now of a mind to consider writing a prescription for it, if the situation warrants it?"
His reply was curt, to the point, and had a somewhat annoyed and confused tone: "I have never written a Paxlovid prescription because...I cannot be sure of the patient's liver function." And that was that. No mention of a survey for the patient, no request for a test for liver enzymes...nothing. He went back to his paperwork, and I promptly directed the patient to the local CVS Minute Clinic.
Once again, I felt defeated. My own clinic, my favorite practitioners, too worried that someone might sue them, and too worried that they might cause harm - to act to give comfort. For those seeking COVID stats, I spent a couple of days compiling areas showing a rise in wastewater and hospitalizations:
Marion County appeared to have a rise in hospitalizations earlier this week. Citrus was also shown. I am now convinced that areas with shared hospitals are being linked, and the counties are not separated by geographical area.
Comparative hospitalizations for COVID treatment between this and last week:
Citrus: 13 new patients, a 62.5% rise, 2.50 patients per 100,000 (above the US average of 2.49). Marion mirrored this.
Indian River: 35 new patients, a 59.1% rise, 4.6 patients per 100,000.
Santa Rosa: 35 patients, a 94.4% rise, 4.2 patients per 100,000. Escambia, Okaloosa, and Walton were linked.
As a control:
Pasco: 130 patients, down 19.8%, 4.1 patients per 100,000.
We do seem to have had more COVID in Pasco, but it is slowly relaxing. That may be short-lived though, considering the holiday.
I have not included any other counties due to the fact that their levels were the same or reduced by a very small level. If you're in any of those fortunate counties, smile! (But still wear your mask.)
Wastewater Levels per the CDC show:
Levi: Significant rise
Alachua: Significant rise
Orange: Tentative rise, just beginning
All other levels were decreasing slowly or unreported.
Memorial weekend and Day will result in people getting together for barbeques, solemn ceremonies, and family outings....and you know more gatherings means more spread. I'll report what I see, of course.
For everyone still masking, keep it up. I've had patients confront me: "Are you comfortable in that mask?" I simply reply that it is the best thing that I can do to guard myself at the clinic....and at the grocery store....and on public transportation. Don't ever apologize. It's your right to stay healthy and happy.
For those dealing with Memorial Day as I do, in remembering loved ones lost to the wars, or just simply lost to time, I am with you.
